Cartilage Replacement in the Knee
A Solution for Maintaining Youthful Movement and an Active Lifestyle
When Is Cartilage Replacement in the Knee Necessary?
Cartilage Deficiency in the Knee – How Does It Develop and Who Does It Affect?
Cartilage deficiency in the knee joint is a condition in which the smooth, elastic cartilage surface covering the bone ends of the knee joint becomes damaged or thinned. This can occur not only in older age, but also in younger, active individuals, particularly when a knee injury has occurred or the joint is subjected to increased stress.
Cartilage tissue does not have its own blood supply, so its regenerative capacity is minimal. When cartilage deficiency presents as a localized, well-defined lesion—such as cartilage detachment or a clearly demarcated cartilage injury—cartilage replacement procedures can be applied. It is important to emphasize, however, that in cases of advanced, diffuse cartilage degeneration, these interventions are not recommended, as this involves a process affecting the entire joint. Cartilage replacement in the knee is particularly suitable for young or middle-aged patients leading active lives for whom knee prosthesis is not yet indicated.
Cartilage Injury During Sports – Common Causes and Consequences
During sports, the knee is particularly exposed to impacts, twisting movements, or sudden changes of direction. In such situations, cartilage surface damage occurring together with meniscus or ligament injury is common. Cartilage injury often does not cause immediate noticeable symptoms, but over time pain, swelling, and restricted range of motion become more frequent, or detached cartilage fragments may cause the joint to “lock.”
For younger athletes, preserving the condition of the cartilage surface is particularly important, as a neglected injury can lead to cartilage deficiency in the long term. In favorable cases, the damaged, detached cartilage is suitable for refixation (e.g., with titanium mini-screws or absorbable mini-pins). Refixation, also known as cartilage refixation, should be performed as quickly as possible. If the detached cartilage is too thin, too small, or has a damaged structure, refixation is no longer possible. In such cases, mosaicplasty—cartilage transplantation using the patient’s own cartilage—or other cartilage replacement procedures can help restore the smooth surface of the joint and enable a return to sports.
Treatment of Knee Cartilage Injury: Conservative Options and Surgical Solutions
Treatment of knee cartilage injury always depends on the severity of the lesion, its location, and the patient’s lifestyle. In cases of milder symptoms, conservative therapy may be sufficient: physiotherapy, cartilage-building dietary supplements, injection treatments (e.g., hyaluronic acid, PRP). The goal of these is to reduce pain and stabilize the condition of the cartilage.
If the cartilage deficiency is more extensive or symptoms do not improve, surgical intervention may be considered. In such cases, the damaged cartilage surface is specifically replaced, typically using mosaicplasty technique or other modern cartilage replacement procedures. The precise treatment plan should always be determined individually, based on a specialist medical examination.

The Role of Cartilage Replacement Surgery
The goal of cartilage replacement is to restore the damaged cartilage surface of the knee joint, thereby eliminating pain, improving range of motion, and preventing further joint degeneration.
What Is Mosaicplasty Surgery?
The surgery (mosaicplasty) is a modern cartilage replacement procedure used to treat smaller, well-defined cartilage defects.
The essence of the method:
- Small cylinders (grafts) are removed together with healthy cartilage and bone from a less weight-bearing area of the knee
- These are transplanted to the damaged area, creating a new, smooth cartilage surface
- The cylinders fit together in a mosaic pattern, hence the name of the procedure
This type of surgery can help avoid prosthesis implantation, especially in younger patients.
How Does Cartilage Replacement Surgery Proceed Step by Step?
The surgical procedure consists of the following steps:
- Preparatory examinations (MRI, X-ray, clinical examination)
- Intervention performed under general anesthesia or spinal anesthesia
- Graft removal from the area around the patellar groove using a small incision or arthroscopy
- Appropriately sized bone holes are created in the cartilage defect area the removed graft is implanted here using press-fit technique, so separate fixation is not necessary.
- Closure of the joint capsule, then the wound layers, sterile dressing
- Rehabilitation under the guidance of physiotherapists begins immediately after surgery
- Rehabilitation takes 6–12 weeks
The type and duration of surgery always depend on the extent of the lesion.
What Cartilage Replacement Options Are Available Today?
In addition to mosaicplasty surgery, other procedures are also available, depending on the size of the damaged area and the condition of the surrounding tissues.
Possible solutions:
- Microfracture/drilling technique – the bone cortex underlying the damaged cartilage surface is opened, allowing stem cells from deeper layers of the bone to reach the surface, initiating cartilage formation in this area
- Cell-based procedures – laboratory cultivation and reimplantation of cartilage cells
- Allograft implantation – replacement with cartilage from a donor
- Combined techniques – e.g., preparation of the cartilage-damaged area, then covering with special material
The appropriate method is always selected by the orthopedic specialist based on the patient’s condition.
Knee Cartilage Surgery for Young Patients – The Goal Is Full Return to Movement
Cartilage replacement often becomes necessary in young patients leading active lives, where the goal is not only to eliminate pain but to restore full mobility.
The Goal of the Intervention: Rehabilitation of Athletes and Active Individuals
Mosaicplasty surgery is primarily performed in cases when:
- The patient actively participates in sports or performs physical work
- There is a smaller (a few cm², well-localized) cartilage defect on the femoral joint surface
- The goal is long-term preservation of the joint and delaying prosthesis implantation
The intervention provides an opportunity to regain full movement, even for sports activities.
How Long Is Recovery After Cartilage Replacement?
Recovery consists of several stages and progresses gradually:
- In the first weeks, weight relief, use of assistive devices
- From weeks 4–6, gradual loading, physiotherapy
- Full weight-bearing typically after 2–3 months
- Tissue integration can take up to 6 months
The exact duration of recovery most often depends on individual factors.
When Can the Patient Return to Sports?
Returning to sports depends not only on physical recovery but also on the restoration of muscle strength, balance, and movement coordination.
General guidelines:
- Lighter sports activities: after 3–4 months
- Running, ball games: after 6 months or later
- Full return is possible after medical checkup and functional tests
It is important that the principle of gradualness is applied during rehabilitation.
