Tunnel syndrome is a painful, numb hand condition that most commonly affects the wrist. It is called Carpal tunnel or carpal tunnel syndrome. Regular, monotonous overuse of the fingers and wrists, as well as hormonal changes, play a major role in its development. For this reason, it is particularly common in middle-aged women. Tunnel Syndrome Surgery is the most effective way to eliminate the pain.


Development of carpal tunnel syndrome

Around the wrist joint of the hand central nerve, and the muscles, tendons, blood vessels and bony formations that run around it, make up the area known as the carpal tunnel.
This tunnel is formed by the U-shape of the hand bones and the bridging ligament. When these the nerves are under pressure and the tunnel becomes narrow, numbness and later pain in the hand.

The development of tunnel syndrome can be aggravated by occupations that put a lot of strain on the wrist and elbow, strenuous activity, diabetes, injuries and, temporarily, pregnancy. The development of Carpal tunnel syndrome is more common in the dominant hand, but can sometimes occur in both hands.

Symptoms of tunnel syndrome

In the initial stages of tunnel syndrome, the first symptoms are unpleasant tingling, numbness you will sign up. The symptoms are typically felt in the first three fingers of the hand, but do not affect the little finger or ring finger. The unpleasant tingling, numbness is felt in the fingers and the thumb side of the palm. The complaint is caused by the inac duzzanata leaves less space for the middle nerve in the narrow hand tunnel in a sewer. This puts pressure on the median nerve and causes the area around it to become numb.

In the absence of appropriate treatment may cause severe, burning pain around the wrists and in the fingers, which can become permanent over time, and then can radiate out the whole arm. The grip of the fingers and palm loosens, the muscles of the palm and the pad of the thumb start to atrophy, causing the hand to become clumsy. The fine movements of the thumb, index and middle fingers also deteriorate. The longer you wait to treat, the more damage is done to nerves, muscles, tendons and soft tissues.

Tunnel syndrome diagnosis

With the complaints described a specialist in hand surgery who will perform targeted tests to clarify the presence and stage of tunnel syndrome. The first element of the hand examination is the anamnesis, i.e. when the patient gives a detailed account of his or her complaints. This is supported by specific questions from the hand specialist. This is followed by a physical examination, where the possibility of tunnel syndrome is raised.

To make an accurate diagnosis electroneurographic (ENG) examination or sometimes the specialist may recommend an X-ray or ultrasound scan of the affected area. An electroneurographic examination gives an idea of the condition of the nerves in the wrist, and therefore the stage of the tunnel disease. On this basis, the specialist will draw up an appropriate treatment plan.

Treatment of tunnel syndrome

In the early stages of the syndrome, the hand surgeon may use various conservative may suggest (non-surgical) treatments. Most commonly, the fixation of the wrist, resting, physiotherapy, possibly physiotherapy treatment. At a moderately advanced stage, steroid injections may be considered. If conservative treatment fails or the diagnosis is made at an advanced stage of the disease, surgical release of the nerves is recommended.

Carpal tunnel syndrome surgery is performed as a day case surgical procedure. The operation can be performed under local anaesthesia or under general anaesthesia; in each case, the specialist will make a recommendation.

During tunnel syndrome surgery, the hand surgeon cuts the ligament to free the narrowed wrist tunnel. This allows more space for the central nerve and flexor tendons of the hand, and relieves the pressure that has built up.

During the operation, the inflamed tissue surrounding the tendons is removed if necessary. Finally, the surgical wound is carefully sutured with a few stitches.

Recovery time after tunnel syndrome surgery

The first post-operative check-up is 1-2 days after the operation, during which the specialist removes the blood drainage tube (drain) from the operated hand.
Suture removal is due 7-14 days after the operation, after which, on the recommendation of the specialist, the physiotherapy the execution and the ultrasound scar treatment.
If the tunnel disease has caused a disturbance of the hand's motor functions, recommended occupational therapy sessions after surgery for a full recovery.
During the rehabilitation days, it is recommended to take larger amounts of Vitamin B which helps the nerves to regenerate. The total recovery time is about two months.

Why is carpal tunnel syndrome surgery important?

The uncomfortable night-time tingling, numbness in the back of the hand is often ignored in the early stages. However tunnel syndrome is characterised by the fact that the symptoms do not decrease over time, but are growing and expanding. Thus, in the absence of appropriate treatment nocturnal symptoms are accompanied by pain, and also during the day with increasing intensity. In the long term, tunnel syndrome has a significant negative impact on the quality and quantity of rest at night, daytime activities, work and other daily routines. Delaying treatment muscles, nerves and tendons start to atrophy, which can cause loss of function in the hands and fingers. The risk of surgical intervention is much lower than the negative impact on quality of life of the consequences of tunnel syndrome.
